 Self Determination is an idea generated by people with developmental disabilities and their families. They wanted more control over their lives and the ability to choose and give direction over the supports and services that would best assist them in achieving the lives they wanted. The major ideas of self determination are Consumer Control and Authority, Life Decision Making, Community Participation and Individual Responsibility.
| Start Up Broker Services |
| CP Rochester provides Start Up Broker services for people enrolling in the Self Determination Program. A Start Up Broker is a person hired by the person with a developmental disability who will assist with completion of a CSS application. A Start Up Broker cannot be the Medicaid Service Coordinator. The Start Up Broker assists the person in developing a Circle of Support, a Person Centered Plan, and the CSS Plan and Budget. The purpose of the Start Up Broker is a help their person put all the prices of a successful CSS Plan application into place. |

| Support Broker Services |
| Once the CSS Plan is approved the Support Broker can assist the person in getting the plan operational. The Support Broker works on specific objectives of the plan, coordination of the Circle of Support, and may assist with the annual plan and budget revisions. CP Rochester provides Support Broker Services. |
