Community Integration Counseling (CIC)

The challenges faced by an individual with Traumatic Brain Injury (TBI) and their family are multiple and life altering.  CP Rochester recognizes the need to support individuals and their families to successfully navigate all the emotional roadblocks and changes in their everyday lives.  Our qualified and caring staff will assist the person with TBI and the family in setting the path toward integrating back into their community to the fullest extent possible.

Service Coordination

A knowledgeable and caring Service Coordinator can provide all the options your community has to offer so you can choose the right supports and services to fit you and your family's needs, when you need them.  Through assessment and discussion of personal goals, a detailed plan is developed and implemented with the support of the Service Coordinator.
