Residential Group Homes throughout the Rochester area (5 Individual Residential Alternative Homes and 1 Respite Home)

Hours Available

Day, evening, weekend shifts vary based on program and location

Job Purpose and Primary Duties

• Ensures all consumer medical and health needs are addressed promptly under the clinical direction of the Registered Nurse
• Schedules and attends medical appointments with consumers and provides follow-up to families, staff and service providers as necessary and appropriate
• Assures that consumer medications and supplies are available and being administered as per physician orders

Qualifications

• NYS licensure and registration as a Licensed Practical Nurse in good standing
• One-year experience in working with developmentally disabled individuals strongly preferred
• Demonstrated ability to problem-solve and prioritize medical, health, and safety situations
• Valid NYS Driver's License required
• Ability to meet all agency and program required training
• Ability to perform all essential job functions
