Residential Group Homes throughout the Rochester area (5 Individual Residential Alternative Homes and 1 Respite Home)
Hours Available
Day, evening, weekend shifts vary based on program and locationApplications and resumes accepted on a rolling basis
Job Purpose and Primary Duties
- Ensures all consumer medical and health needs are addressed promptly under the clinical direction of the Registered Nurse
- Schedules and attends medical appointments with consumers and provides follow-up to families, staff and service providers as necessary and appropriate
- Assures that consumer medications and supplies are available and being administered as per physician orders
Qualifications
- NYS licensure and registration as a Licensed Practical Nurse in good standing
- One-year experience in working with developmentally disabled individuals strongly preferred
- Demonstrated ability to problem-solve and prioritize medical, health, and safety situations
- Valid NYS Driver's License required
- Ability to meet all agency and program required training
- Ability to perform all essential job functions
