Augustin Children's Center

Augustin Children’s Center provides a full continuum of educational and therapeutic services for children, birth to kindergarten, with and without disabilities.  Individualized programs and attention from a highly-trained, experienced staff are delivered where your child spends his/her day or on our campus in an ideal, accessible suburban setting featuring a gymnasium, playground and a heated swimming pool.  Our renowned center seamlessly integrates children with disabilities and typical kids in a unique and rewarding educational environment.

Early Intervention Program

Location: Determined by the Individualized Family Service Plan (IFSP) and may be provided in childcare and community settings.  Parents are responsible for paying costs associated with that setting.

The Early Intervention Program is a public program funded by NYS and Monroe County for children under the age of 3 who have an eligible diagnosis or developmental delay.  To determine whether or not your child is eligible, contact the Monroe County Early Intervention Program at (585)753-5437.  Eligibility will be determined by a NYS approved evaluator contracted by Monroe County.  If your child is eligible, services will be implemented by Monroe County in collaboration with you and at no cost to you, but your health insurance may be accessed for reimbursement.

This individualized combination of educational and therapeutic services for infants under age 3 are specified in the Individualized Family Service Plan (IFSP).  Services provided by CP Rochester may include Special Education, Speech, Physical Therapy, Occupational Therapy, Parent Training, Social Work and Support Groups.  Services can be provided individually or in our small group setting.

Serves: Infants, birth through age 2, and their families who are eligible for Early Intervention Services.

Preschool Program

Augustin Children’s Center offers a 5-half day integrated preschool program to all children, with or without disabilities, from 3 yrs old to Kindergarten age.  A Masters degreed Special Education Teacher leads each team.  Our team members include a Speech Pathologist, Physical Therapist, and Occupational Therapist and 2 assistants.  A Music Therapist and an Adaptive Physical Education Teacher provide additional support and service.   Special services are provided to children with disabilities as indicated in each child’s Individualized Educational Plan (IEP) either as part of their preschool program or as Related Services Only with or without Special Instruction Education Services (SEIT).

Serves: Children aged 3 yrs to Kindergarten, with and without disabilities.  Augustin Children’s Center is also an approved Rush-Henrietta Universal Pre-Kindergarten site.

Structured Day Program

Individuals who have experienced a traumatic brain injury can expect to receive support to develop their own personal detailed plan outlining short and long term goals to reach a desired level of independence in life.  Caring and highly trained professionals will provide a variety of options and cognitive strategies to fit each individual's learning style in a comfortable atmosphere of education.  Clinical services can be integrated into the day for an enhanced learning and skill building experience.

Community Integration Counseling (CIC)

The challenges faced by an individual with Traumatic Brain Injury (TBI) and their family are multiple and life altering.  CP Rochester recognizes the need to support individuals and their families to successfully navigate all the emotional roadblocks and changes in their everyday lives.  Our qualified and caring staff will assist the person with TBI and the family in setting the path toward integrating back into their community to the fullest extent possible.

Service Coordination

A knowledgeable and caring Service Coordinator can provide all the options your community has to offer so you can choose the right supports and services to fit you and your family's needs, when you need them.  Through assessment and discussion of personal goals, a detailed plan is developed and implemented with the support of the Service Coordinator.

Outpatient Clinic Services

Pediatric Therapy Services

Pediatric Therapy Services (PTS) is an outpatient clinic for children, birth through age 18, in and around Monroe County.  PTS provides evaluations and treatment for a wide variety of diagnoses and rehabilitation needs.  Services available include occupational therapy, physical & aquatic therapy, speech and language therapy, music therapy, infant massage, and assistive technology services. Therapy can be provided at PTS or in the school setting, when authorized by the school district.

Occupational Therapy

Occupational Therapy (OT) facilitates the maximum level of independence through the assessment of fine motor skills, cognitive skills, visual skills, and the individual’s ability to perform activities of daily living (ADL’s). Therapy is individualized, based on an individual’s evaluation and can focus on:

  • the relaxation of muscle tone (through modalities such as heat, massage, paraffin, baths, vibration, etc)
  • increasing upper extremity range of motion
  • improving gross/fine motor coordination
  • increasing perceptual/cognitive and academic skills
  • increasing independent living skills or ADL’s through the use of adaptive equipment or re-education

The Occupational Therapists also perform assessments and develop appropriate home programs for sensory dysfunction.

We also have a Certified Hand Therapist (CHT) as part of our Occupational Therapy team.

Physical Therapy

Pediatric Physical Therapists evaluate and treat physical disability due to disease, injury, and/or birth defect. PT addresses posture, mobility, strength, balance, coordination, flexibility, normal motor development, and ability to perform functional activities of daily living. Conditions that may benefit from physical therapy are Torticollis and Positional Plagiocephaly, Cerebral Palsy, Brachial Plexus Injury, and Progressive Neuromuscular Diseases.

 

Aquatic Therapy

The heated therapy pool offers physical therapy in an alternative environment.  The warmth and buoyancy of the water promote movement when it is too difficult to exercise on land.  The pool increases strength, decreases pain, allows for partial weight bearing and ambulation, strengthens respiratory muscles, and promotes independence.

Speech/Language Therapy

Speech/Language Pathologists (SLP’s) are responsible for the diagnosis and treatment of speech, language and/or swallowing disorders.  Speech Therapy (ST) is provided to individuals exhibiting communication disorders primarily in the areas of language (grammar and memory), articulation, voice production, fluency, and aural rehabilitation (hearing).  Evaluation and training for the use of augmentative communication (speech generating devices) is also provided.  The primary goal is to enhance functional communication skills.  Additional services include evaluation and recommendations for feeding and swallowing difficulties (dysphagia).  All pathologists in the speech department are board certified by the American Speech-Language and Hearing Association.

Music Therapy

Music Therapy facilitates greater independence by using music as a motivational tool.  Additional incorporated techniques include creative arts such as drama, dance and artistic expression.  Music therapy can also address the following domains:

  • motor skills
  • communication
  • cognition
  • social
  • affective and auditory memory

Our Music Therapist is Board Certified (MT-BC) and a LCAT (Licensed Creative Arts Therapist).

Infant Massage

Infant massage promotes bonding and interaction between the parent and infant.  Infant massage provides relaxation, helps relieve discomfort from constipation, gas, and colic, improves sleep, and improves the parents’ awareness of babies’ cues.  This class teaches the parent how to perform massage while allowing one on one time with their baby.  For more information on classes click here!

Our behavioral health services include Psychiatry, Psychology and Clinical Social Work.  The department addresses various psychosocial, behavioral, emotional and mental health needs of patients.  This is done through evaluations, diagnosis, assessment, ongoing counseling and medication management.  Assessments for adaptive functioning, intelligence and decision making abilities are also offered.  All members of the behavioral health team are licensed professionals and develop services based on individual needs.

Horseback Riding

As part of a complete recreation program, CP Rochester offers a recreational - Learn to ride program for people with special needs.  Our riding program is community based and offers the participants an opportunity to gain skills and experience

Location: Sites vary – we are  currently running our program with the support of Meadows of Mendon Stables – West Bloomfield Road

Serves: Programs vary with each quarterly class session

Additional: three six week sessions are offered annually  

Swimming

Location: 1000 Elmwood Ave or 3399 Winton Road

We offer a variety of swim program to meet the needs of the individuals and families that we serve.  Programs include 1:1 lessons, group swim, family swim and open swim programs.

Location: Swain Ski Center, Swain, NY

A seasonal program that runs from January through March. Shared Ski Adventures is an 8 week learn to ski program for people of all ages with special needs.  Adaptive equipment and our trained instructors make downhill skiing a possibility for everyone.

Additional: This program relies heavily on the support of skilled volunteers – program size depends upon the availability of volunteer support

As part of a complete recreation program, CP Rochester offers a variety of community based outing for families and individuals of all ages.  Venues include Performing Arts, Sporting Events, Cultural Opportunities, Movies and Dinner trips.  Such outings provide opportunities to socialize and network as well as a chance to participate in the local community events in an affordable fashion. Adaptive equipment and trained staff make bowling possible for everyone.

Bowling

As part of our complete recreation program, CP Rochester offers 8 week bowling leagues three times throughout the year for adults to gain skills, socialize and have fun.

Assistive Technology Evaluation Center

(Note: We can also go to the individual’s residence or work site).

The Assistive Technology Evaluation Center is designed as a studio apartment which provides a home like setting in which the multi-disciplinary team can evaluate an individuals abilities.  The clinical team can assess individuals with physical and developmental disabilities in their choice to lead an active and productive life as part of their community.  The evaluation center provides opportunities for people with physical disabilities to experience assistive and adaptive technology that will enhance their independence and quality of life.

Additional aspects that the evaluation addresses includes: providing professional team-based evaluation and training at the apartment or consumer’s residence advising consumers, their families and caregivers, and service coordinators of the assistive and adaptive technologies available to them for their particular day-to-day needs

In addition to third party insurances, this program receives funding through the United Way, Vocational and Educational Services for Individuals with Disabilities (VESID) and Options for People Through Services (OPTS).
