         <title>Assistive Technology Evaluation Center</title>

Location: 95 Pinnacle Road (close proximity to our main site at 3399 Winton Road South)<br />
(Note: We can also go to the individual&rsquo;s residence or work site).</p>
<p>The Assistive Technology Evaluation Center is designed as a studio apartment which provides a home like setting in which the multi-disciplinary team can evaluate an individuals abilities.&nbsp; The clinical team can assess individuals with physical and developmental disabilities in their choice to lead an active and productive life as part of their community.&nbsp; The evaluation center provides opportunities for people with physical disabilities to experience assistive and adaptive technology that will enhance their independence and quality of life.<br />
<br />
Additional aspects that the evaluation addresses includes: providing professional team-based evaluation and training at the apartment or consumer&rsquo;s residence advising consumers, their families and caregivers, and service coordinators of the assistive and adaptive technologies available to them for their particular day-to-day needs<br />
<br />
In addition to third party insurances, this program receives funding through the United Way, Vocational and Educational Services for Individuals with Disabilities (VESID) and Options for People Through Services (OPTS).</p>]]></description>

         <title>Community Recreation</title>

<strong>Horseback Riding</strong></p>
<p>As part of a complete recreation program, CP Rochester offers a recreational - Learn to ride program for people with special needs.&nbsp; Our riding program is community based and offers the participants an opportunity to gain skills and experience<br />
<br />
Location: Sites vary &ndash; we are&nbsp; currently running our program with the support of Meadows of Mendon Stables &ndash; West Bloomfield Road<br />
<br />
Serves: Programs vary with each quarterly class session<br />
<br />
Additional: three six week sessions are offered annually&nbsp;&nbsp;</p>
<p><strong>Swimming</strong><br />
<br />
We offer a variety of swim program to meet the needs of the individuals and families that we serve.&nbsp; Programs include 1:1 lessons, group swim, family swim and open swim programs.<br />

Location: Sites Vary 1000 Elmwood Ave or 3399 Winton Road</p>
<p><strong>Skiing<br />
</strong><br />
A seasonal program that runs from January through March. Shared Ski Adventures is an 8 week learn to ski program for people of all ages with special needs.&nbsp; Adaptive equipment and our trained instructors make downhill skiing a possibility for everyone.<br />

Location: Swain Ski Center &ndash; Swain New York<br />
<br />
Additional: This program relies heavily on the support of skilled volunteers &ndash; program size depends upon the availability of volunteer support&nbsp;</p>
<p><strong>Special Trips<br />
</strong><br />
As part of a complete recreation program, CP Rochester offers a variety of community based outing for families and individuals of all ages.&nbsp; Venues include Performing Arts, Sporting Events, Cultural Opportunities, Movies and Dinner trips.&nbsp; Such outings provide opportunities to socialize and network as well as a chance to participate in the local community events in an affordable fashion. Adaptive equipment and trained staff make bowling possible for everyone.</p>

As part of our complete recreation program, CP Rochester offers 8 week bowling leagues three times throughout the year for adults to gain skills, socialize and have fun.</p>]]></description>

         <title>Outpatient Clinic Services</title>
         <description><![CDATA[<p><strong>Individual Health Services</strong></p>

Location: 3399 Winton Road South (main site), 1000 Elmwood Avenue (satellite site), and part-time clinic sites in the Rochester area</p>
<p>Individual Health Services is an outpatient clinic providing evaluations and services for adults age 18 through the senior years.&nbsp; IHS offers therapeutic programs for a wide variety of diagnoses and rehabilitation needs.&nbsp; Services available include occupational therapy, physical and aquatic therapy, speech and language therapy, assistive technology services and behavioral health services including psychiatry, psychology and social work.<br />
<br />
<strong>Occupational Therapy</strong><br />
<br />
Occupational Therapy (OT) facilitates the maximum level of independence through the assessment of fine motor skills, cognitive skills, visual skills, and the individual&rsquo;s ability to perform activities of daily living (ADL&rsquo;s).&nbsp; Therapy is individualized, based on an individual&rsquo;s evaluation and can focus on:<br />
<br />
the relaxation of muscle tone (through modalities such as heat, massage, paraffin, baths, vibration, etc)<br />
increasing upper extremity range of motion<br />
improving gross/fine motor coordination<br />
increasing perceptual/cognitive and academic skills<br />
increasing independent living skills or ADL&rsquo;s through the use of adaptive equipment or re-education<br />
<br />
The Occupational Therapists also perform assessments and develop appropriate home programs for sensory dysfunction.<br />
<br />
We also have a Certified Hand Therapist (CHT) as part of our Occupational Therapy team.<br />
<br />
<strong>Physical Therapy</strong><br />
<br />
Pediatric Physical Therapists evaluate and treat physical disability due to disease, injury, and/or birth defect. PT addresses posture, mobility, strength, balance, coordination, flexibility, normal motor development, and ability to perform functional activities of daily living. Conditions that may benefit from physical therapy are Torticollis and Positional Plagiocephaly, Cerebral Palsy, Brachial Plexus Injury, and Progressive Neuromuscular Diseases.<br />
<br />
<strong>Aquatic Therapy</strong><br />
<br />
The heated therapy pool offers physical therapy in an alternative environment.&nbsp; The warmth and buoyancy of the water promote movement when it is too difficult to exercise on land.&nbsp; The pool increases strength, decreases pain, allows for partial weight bearing and ambulation, strengthens respiratory muscles, and promotes independence<br />
<br />
<strong>Speech/Language Therapy</strong><br />
<br />
Speech/Language Pathologists (SLP&rsquo;s) are responsible for the diagnosis and treatment of speech, language and/or swallowing disorders.&nbsp; Speech Therapy (ST) is provided to individuals exhibiting communication disorders primarily in the areas of language (grammar and memory), articulation, voice production, fluency, and aural rehabilitation (hearing).&nbsp; Evaluation and training for the use of augmentative communication (speech generating devices) is also provided.&nbsp; The primary goal is to enhance functional communication skills.&nbsp; Additional services include evaluation and recommendations for feeding and swallowing difficulties (dysphagia).&nbsp; All pathologists in the speech department are board certified by the American Speech-Language and Hearing Association.<br />
<br />
<strong>Music Therapy</strong><br />
<br />
Music Therapy facilitates greater independence by using music as a motivational tool.&nbsp; Additional incorporated techniques include creative arts such as drama, dance and artistic expression.&nbsp; Music therapy can also address the following domains:<br />

motor skills<br />
communication<br />
cognition<br />
social<br />
affective and auditory memory<br />
Our Music Therapist is Board Certified (MT-BC) and a LCAT (Licensed Creative Arts Therapist).<br />

<strong>Behavioral Health Services</strong></p>

<p>Our behavioral health services include Psychiatry, Psychology and Clinical Social Work.&nbsp; The department addresses various psychosocial, behavioral, emotional and mental health needs of patients.&nbsp; This is done through evaluations, diagnosis, assessment, ongoing counseling and medication management.&nbsp; Assessments for adaptive functioning, intelligence and decision making abilities are also offered.&nbsp; All members of the behavioral health team are licensed professionals and develop services based on individual needs.</p>]]></description>
