CP Rochester owns and manages four Individualized Residential Alternatives in Monroe County. Each residence is unique and compassionately supports a wide range of individuals with special needs. All the homes are fully staffed day and night, providing individualized assistance and skill-building toward personal goals using capacities and preferences in a family-style setting. Brighton - Elmwood Avenue Irondequoit - Kings Highway Perinton/Pittsford - Benedict Road Pittsford - Jefferson Road In addition to CP Rochester’s four residences, it also owns and manages a certified free-standing respite facility in Henrietta (95 Pinnacle Road).     About Us Adult Services Children Services Employment News Events Foundation More About Us Adult Services Children Services Employment News Events Foundation More Donate Now Self-Direction Services (SDS) Available to individuals enrolled in the HCBS waiver who want the freedom to choose supports and services that are right for them. With the assistance of a Fiscal Intermediary and Support Broker, individuals have the flexibility to manage their budget and have control over their funding. As the OPWDD website states, “Self-Direction gives you the chance to make decisions about supports and services that help you to: Enjoy meaningful relationships with family and friends, Experience health and growth, Live in the home and community you choose, and Work, volunteer or do activities you enjoy. Enhancing community integration and social capital is at the core of self-direction.” Fiscal Intermediaries Fiscal Intermediaries provide administrative and billing support to self-directed individuals, including but not limited to completing staff background checks, processing staff payments, billing for Medicaid services, and tracking expenditures. Support Brokers Support Brokers help individuals to exercise their freedom in choosing supports and services. The support broker will request an initial start-up budget, and upon approval, will help to plan and develop a complete Self-Direction budget. They provide ongoing support to individuals by building their Circle of Support team, providing education on services and community resources, training staff, and more. Ability Partner agencies provide a lifetime of inclusive opportunities, ensuring that people of all abilities thrive. The agencies play critical roles in the economic well-being of the communities in which they operate, employing over 700 staff and serving 6,800 individuals and families in 20 locations in the Greater Rochester and Finger Lakes regions. Over the past year, your support has enabled us to provide essential services post-pandemic as well as amid a natural disaster, expand crucial educational and mental health services, launch counseling services for children facing anxiety and depression, as well as address and adapt to evolving community challenges.     Children Services The Augustin Children’s Center is an accessible, modern, and well-equipped facility. The preschool caters to children with and without disabilities ages three to five, and it offers early intervention services for ages two and under. The Center boasts many features not offered by other providers such as a warm-water indoor pool, a large gymnasium with a stage, an outdoor exploratory classroom, a large playground with a ‘safe surface,’ and fully integrated classrooms and programs. Offering support and guidance to the entire family, CP Rochester’s Augustin Children’s Center builds a foundation for success in a nurturing environment through quality education and therapeutic services. Early Intervention Early Intervention : Services such as special education, speech, social work, and occupational/physical therapy to children with a wide range of disabilities from birth to age three. Parents, teachers, and therapists work together to create individual plans for each child. Special Education : Our teachers use targeted games, activities, and play techniques to help children engage in the world around them. Speech Therapy : Therapists use sign language, vocabulary development, and articulation training to help children build communication skills. They also work with children with eating and swallowing difficulties. Physical Therapy : Allows children to work on muscle strength, coordination, flexibility, and mobility. Our warm water pool (94 degrees) helps children achieve maximum range of motion while building core strength, tone, and balance. Plus, it’s fun! Occupational Therapy : Helps develop the skills necessary to be successful in fine motor, self-help, and sensory skills. This may include activities such as feeding and dressing, manipulating toys, pre-writing, and being able to take in and respond appropriately to stimuli in the environment. Home/Community Services : To best support families and understand how children interact with their home environment, therapists, teachers, and parents work as a team providing services at home, daycare, and other community settings. Family Services : One of the important goals of early intervention is working with the entire family for the benefit of the child. All parents can participate in individual counseling sessions to help manage challenges. A master’s level social worker helps families access other needed community programs and resources. The Early Intervention Program is funded through New York State and Monroe County for children under the age of three who have an eligible diagnosis or developmental delay. To determine your child’s eligibility, contact the Monroe County Early Intervention Program at 585-753-5437. Eligibility will be determined by a NYS-approved evaluator contracted by Monroe County. If your child is eligible, services will be implemented at no cost to you. Programs are designed to help all children take full advantage of their educational and growth potential. The developmentally-appropriate curriculum addresses each child’s individual needs. Activities are properly adapted to each child’s abilities and interests, supporting the areas of communication, physical, social, emotional, and cognitive growth. Children are given the opportunity to imagine, learn, and problem-solve within a stimulating and challenging environment. Experiences in art, music, reading, language, social development, large/small muscle development, as well as a nutritionally-balanced snack are offered. Independence is always encouraged. We offer integrated preschool opportunities designed to meet the intensive, individual needs of children with multiple diagnoses Spectrum Disorder. Integrated Preschool As a universal pre-kindergarten provider, CP Rochester’s Augustin Children’s Center provides integrated preschool services to children with and without disabilities. Each classroom is led by a special education teacher with the assistance of classroom aides. We strive for a 1:4 adult-to-child ratio. We offer music sessions through our licensed music therapist. For those children in our preschool program who require therapeutic services, skilled therapists conduct sessions directly in the classroom whenever possible, integrating with the child’s educational experience and allowing the child to maintain a presence with their peers. By being fully integrated, children with disabilities can model their peers' age-appropriate behavior with access to higher learning standards. Children without disabilities have access to a language-enriched, therapeutic learning environment and gain an appreciation of diversity and differences. Family Services : One of the important goals of early intervention is working with the entire family for the benefit of the child. All parents are offered the opportunity to participate in the Augustin Children’s Center’s monthly parent Support Group as well as individual counseling sessions to help manage challenges. Community Habilitation A person-centered, outcome-driven service provided in the home and community, which provides support and assistance to individuals to increase independence. Various goals are chosen and implemented by the individual with staff support, including shopping for and preparing meals, budgeting, and completing household chores. Please note that we also provide collegiate supports when appropriate for individuals pursuing higher education. These supports include attending class with the individual, taking notes, and organizational/time management skills. Hourly Respite Services Our Respite programs are designed to assist and provide relief to families and caregivers members who are responsible for the primary care and support of individuals with developmental disabilities. There are 3 different types of non-certified respite services: In Home Respite, Recreation Respite, and Wrap Around Respite. In-Home Respite provided in the individual’s home for approximately 2-6 hours per week. Trained, trusted staff comes to the home to meet the needs of the individual and engage the individual in preferred activities. Caregivers have the opportunity to meet staff before starting service. Site-Based / Recreation Respite provides opportunities for individuals with disabilities to develop lifetime recreation skills and interests. What makes CP Rochester unique is that most of our programs and events take place in the community. We offer opportunities for individual and group interaction and programs incorporating instruction and skill development such as individual swimming lessons, fitness, and social opportunities. Wrap Around Respite The Wraparound Respite Program (for individuals 12+) runs Monday – Friday from 2:30 PM – 5:30 PM at CP Rochester’s Winton Road location.
