Demonstrated ability to effectively communicate orally and in writing. Ability to complete required training courses included but not limited to First Aid, CPR and Medication Administration. Imparts agency philosophy of “Respect, Integrity, Service and Excellence” to individuals, families, program staff and community resources. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy. Creates opportunities for, and encourages individuals to develop skills in recreation/leisure/hobbies, home management, social/interpersonal and other skill areas including the promotion of self-respect and being respectful of others while developing relationships in the community. Follows consumer activity and behavioral guidelines as set by the team and desired outcome guidelines. These may include but not be limited to Residential Habilitation Plans and IPOPs. Ensures consumer care meets or exceeds established standards in areas including personal hygiene, grooming, feeding, care and respect for consumer belongings, room cleaning, and other daily living skills. Completes documentation as required by and in coordination with regulations and agency policies and procedures. Adheres to the agency’s policies and procedures as well as department guidelines. Performs other duties at the discretion of the Program Director or his/her designee. Physical Requirements include: sitting, standing, walking, lifting over 50 pounds, pulling and pushing wheelchairs and adaptive equipment, carrying, grasping, reaching, stooping and crouching, crawling, color determination, visual acuity, speaking clearly, listening to spoken communication, sense of taste and smell. Speech-Language Pathologists are responsible for the diagnosis and treatment of speech, language and/or swallowing disorders. Speech-language therapy is provided to individuals exhibiting communication disorders primarily in the areas of language (grammar and memory), articulation, voice production, fluency, and aural rehabilitation (hearing). Evaluation and training for the use of augmentative communication (e.g. speech generating devices) are also provided. The primary goal is to enhance functional communication skills. Additional services include evaluation and recommendations for feeding and swallowing difficulties (dysphagia). All pathologists in the Speech Department are board certified by the American Speech-Language and Hearing Association. With the assistance of a Fiscal Intermediary and Support Broker, individuals have the flexibility to manage their budget and have control over their funding. As the OPWDD website states, “Self-Direction gives you the chance to make decisions about supports and services that help you to: Enjoy meaningful relationships with family and friends, Experience health and growth, Live in the home and community you choose, and Work, volunteer or do activities you enjoy. Enhancing community integration and social capital is at the core of self-direction.” Fiscal Intermediaries Fiscal Intermediaries provide administrative and billing support to self-directed individuals, including but not limited to completing staff background checks, processing staff payments, billing for Medicaid services, and tracking expenditures. Support Brokers Support Brokers help individuals to exercise their freedom in choosing supports and services. The support broker will request an initial start-up budget, and upon approval, will help to plan and develop a complete Self-Direction budget. They provide ongoing support to individuals by building their Circle of Support team, providing education on services and community resources, training staff, and more. December 5, 2023 at 10:00:00 PM My name is Susie and I joined Venture PROS (Rochester Rehabilitation) in September of 2019. It took me a lot to admit that I needed more help than I was getting, but I’m so thankful every day that I did. Everyone from the advisors to the office team supported me every step of the way. I think the classes that most benefited me were codependency and assertiveness. I am able to use my voice in ways I have never been able to, speaking up for not only myself but others as well. I was able to become gainfully employed and then got into management at my previous job. At my new job I have made a lot of headway in the company, becoming the leader of my team within the first year there. I am working on lessening the stigma that comes with talking about mental health and creating a more open environment. I’ve even managed to win a few achievement awards! This year's ambassador will be Zachary Sands, 25, who's a participant in the CP Rochester Day Habilitation Program and a Special Olympics athlete. Zach was diagnosed with cerebral palsy and epilepsy as a child. His determination has helped him achieve his goals with his participation in the Special Olympics. "It's important to raise the needed funds that we have to get to keep our services going and provide programs to the folks who look to us for help," said the President and CEO of Ability Partners Foundation, Darrell Whitbeck. About Us Adult Services Children Services Employment News Events Foundation More About Us Adult Services Children Services Employment News Events Foundation More Donate Now Ability Partners Foundation’s 5K race to benefit CP Rochester, Happiness House, Rochester Rehab December 7, 2023 at 10:00:00 PM ROCHESTER — The Ability Partners Foundation, which includes CP Rochester, Happiness House, and Rochester Rehabilitation, is taking strides for people of all abilities Oct. 14 as the 18th Annual 5K Run for Fun/1M Walk for Wellness returns to CP Rochester. Zachary Sands, 25, will be this year’s run ambassador. A participant in the CP Rochester Day Habilitation Program and a Special Olympics athlete, Zach is known by family and friends as driven, genuine and kind. He adores his friends at CP Rochester and admires and idolizes his staff, who have become mentors. He was diagnosed with cerebral palsy and epilepsy as an infant and has hemiparesis, a condition that makes one side of his body smaller and weaker. Despite these physical challenges, Zach pushes to participate in as many sporting events as he can. His determination has helped him achieve his goals both at his program and with his participation in Special Olympics. This year, runners/walkers/strollers will be treated to Halloween activities as well as an agency open house and vendor fair immediately following the run. Participants and volunteers alike are encouraged to come dressed in costume. The certified race begins and ends at CP Rochester’s main campus at 3399 Winton Road South in Henrietta. This family-friendly event also offers a one-mile walk that takes place in the neighborhood adjacent to the race. Online registration closes at noon Oct. 12. Walk-ups will be allowed the day of the event, but are asked to come at the start of check-in/registration at 8 a.m. For a $30 registration fee, runners will be eligible to compete for top prizes as well as receive a race t-shirt and snacks.
